GRAFT · Guiding Readiness, Access & Fairness in Transplantation
GRAFT builds on transplant-access efforts already underway, helping transplant programs put existing and emerging approaches into action, so modifiable caregiver and social-support barriers don't stop patients from completing transplant evaluation.
What it involves
Under GRAFT, awareness campaigns bring patients, families and partners together around what's changing in care, and Implementation Support Programs find what works and scale it rapidly.
Campaigns can be sponsored: sponsors fund reach and content, never conclusions. Events and publications are promoted, not sponsored. Implementation Support Programs are sponsored separately and follow a defined protocol. Sponsors may help identify implementation challenges, populations or outcomes of interest.
