How AHIS collects, uses and protects personal information on ahis.org.

This Privacy Policy explains how the Alliance for Health Implementation Science, Inc. ("AHIS," "we," "our" or "us") collects, uses and protects personal information when you visit ahis.org or contact us through it. AHIS is an independent nonprofit organization. Questions about this policy can be sent to info@ahis.org.
We collect information you choose to submit through our forms or by email.
Member profile. Name, credentials, title, organization, email, optional headshot, your role, care areas, initiatives, how you would like to take part, your perspectives, financial relationships you choose to disclose, and whether you would like to be considered for a Member Spotlight. We use this information to welcome you as a member, connect you with relevant work, support appropriate transparency in AHIS activities and, if you opt in, prepare a Member Spotlight for your approval.
Identify a gap. A description of the care gap, who it affects, where, care area, type of gap, your role, organization if provided, name and email. We use this information to review the gap and follow up where an Implementation Support Program could help.
Membership inquiry. Name, organization, email and membership category. We use this information to respond about membership.
Affinity Group interest list. Name, email, care area, your role and an optional gap. We use this information to contact you about the Affinity Group and related opportunities to participate.
Newsletter subscription. Email address. We use this information to send Advocacy Ahead, the Access Review and other AHIS updates you requested.
Discovery call request. Name, organization, email and what you would like to discuss. We use this information to schedule and prepare for the call.
When you visit the site, certain technical information may be collected automatically by the site and the service providers that support it. This may include information such as your IP address, browser or device information, pages requested and similar technical information used to operate, secure and understand use of the site.
AHIS does not sell personal information submitted through the site or browsing information.
Our website forms are not intended to collect personal health information. Please do not submit your own or another person's diagnoses, test results, treatment information or other identifiable health information through these forms.
If we become aware that personal health information was submitted through a website form that was not intended to collect it, we will handle it in accordance with applicable law and our applicable retention and deletion obligations. AHIS does not provide medical care, and nothing on this site is medical advice.
We use personal information collected through the website to respond to you, manage memberships, review care gaps and program interest, communicate about AHIS activities, provide communications you requested, prepare Member Spotlights you opted into, operate and secure the website, and support AHIS's activities.
A Member Spotlight is prepared only if you opt in. AHIS determines who is featured.
Before publication, we send you the proposed Spotlight for review and approval. If you designate an organizational reviewer, we also obtain that reviewer's approval before publication.
You may withdraw your approval for future use by emailing info@ahis.org. After receiving your request, AHIS will stop new uses of the Spotlight and take reasonable steps to remove it from channels AHIS controls. Withdrawal does not undo uses or distribution that occurred before your request, and AHIS may be unable to remove copies downloaded, archived or shared by others.
We may share personal information in the following circumstances:
We do not provide personal information submitted through this website to sponsors or funders for their own use unless you direct or authorize us to do so or we tell you otherwise when the information is collected.
Information collected through an Implementation Support Program is governed by the notices, consents or authorizations, protocols, data-use terms and other documents applicable to that program. This Privacy Policy does not expand any party's rights to use identifiable participant or health information.
We retain personal information for as long as reasonably needed for the purposes for which it was collected and for applicable legal, contractual, governance, security and recordkeeping obligations.
If you unsubscribe from a newsletter, we will stop sending communications covered by your request and may retain limited information needed to record and honor the unsubscribe.
You may ask to access, correct or delete personal information AHIS holds about you by emailing info@ahis.org. We may need to verify a request before acting on it, and some information may need to be retained where permitted or required by applicable law or other binding obligations.
Where processing is based on your consent, you may withdraw that consent for future use. Withdrawal does not affect uses that occurred before withdrawal.
Every newsletter includes an unsubscribe mechanism. Depending on where you live and the circumstances, you may have additional rights under applicable privacy laws. AHIS will honor those rights where required.
AHIS uses reasonable administrative, technical and physical safeguards appropriate to the personal information it handles. No method of transmission or storage is completely secure.
This site is not intended for children under 13, and AHIS does not knowingly collect personal information from children under 13 through the website. If we learn that we have done so, we will take appropriate steps consistent with applicable law.
The site may link to or use services provided by other organizations. When you leave ahis.org or interact directly with another organization's service, that organization's privacy practices may apply.

We may update this Privacy Policy from time to time. When we do, we will update the "Last updated" date above.
